Liability Waiver *

The undersigned acknowledges the existence of certain inherent risks in this type of activity and herby agrees to assume all risks. While every effort will be made on the part of TRU Taekwondo Center to make the events and facilities as sage as possible, I realize that any physical activity has the potential for injury and I waive any claim of liability resulting from personal injury and/or loss of personal property against TRU Taekwondo Center, its management, assigned or contracted instructors, students, agents, guests or representatives. Occasional photos or videos of class or special events may be taken. Such material may be used as advertising media and remain the property of TRU Taekwondo Center, LLC and the undersigned waives claim to all model rights whether active or passive. Parent herby stipulates that the participant is physically sound and has medical approval to proceed with this type of event.
