I, the undersigned, acknowledge the existence of certain inherent risks in this type of training and hereby agree to assume all risks. I hereby relieve TRU Taekwondo Center, LLC, its management, instructors, agents, guests, or representatives from any liability resulting from personal injury and or loss of property. All participants must have valid medical insurance to attend trial or regular classes. The student acknowledges that rules and regulations governing this institution have been adopted agrees to abide by all such rules and regulations. Occasional photos or videos of class or special events may be taken. Such material may be used as advertising media and remain the property of TRU Taekwondo Center, LLC, and the undersigned claim to all model rights whether active or passive. I herby acknowledge that the above student/candidate(s) is/are physically sound and has the medical approval to proceed with this type of training.
